Usually, my cover letters are very simple. I address the place i'm submitting to (maybe even a specific editor if they request it), then I put a third-person bio that lists my publication history and then I say something to the effect of "She hopes you consider her piece "..." for publication in your forthcoming issue/edition/etc." and then I close with a "Sincerely..."
